One that gets neither tends to fail on the coldest night of the year.
A furnace in Lancaster County runs hard from November into March. Over a winter it cycles thousands of times, and every cycle heats and cools the burners, the heat exchanger, the ignitor, and the blower motor. Parts wear, dust settles on the flame sensor, the filter loads up, and the burner flame slowly drifts out of adjustment. None of that is visible from the thermostat. An annual visit resets all of it before the season starts, which is why the timing matters as much as the frequency.

Is there ever a reason to service more than once a year? Occasionally. Three things, and they are worth separating because homeowners usually think of only the first one.
Efficiency. A furnace with clean burners, a clean flame sensor, a fresh filter, and a properly adjusted gas pressure delivers the heat it was designed to deliver for the fuel it burns. As those things drift, the furnace needs more gas and longer run times to hold the same temperature, and the gas bill climbs a little each winter without anyone noticing why. A tune-up recovers most of that loss on a healthy furnace. It cannot make a twenty-year-old furnace new, but it can keep a ten-year-old furnace from running like one.
Safety. This is the reason annual service is not optional on a gas furnace. The heat exchanger is the metal wall that keeps combustion gases separate from the air the blower sends into your rooms. When it cracks, carbon monoxide can cross into that air. Cracks are not visible from outside the cabinet, they do not announce themselves, and a home carbon monoxide detector is a last line of defense, not a substitute for inspection. A technician checks the exchanger, tests the flame, confirms the venting is drawing properly, and measures carbon monoxide at the registers. Skipping that check for a few years to save on a service visit is a bad trade.
Warranty. Most furnace manufacturers require documented annual professional maintenance as a condition of the parts warranty, and they can deny a claim on a failed heat exchanger or control board if there are no service records. Change the filter every one to three months. This is the single most important thing you can do for a furnace, and the most commonly skipped. A loaded filter starves the furnace of air, which makes it overheat, trip its high-limit switch, and short cycle, and over a season it can crack a heat exchanger. Thin one-inch filters need changing about monthly during heating season; thicker four- or five-inch media filters last longer. Check it once a month regardless, and if you cannot see light through it, replace it. Write the date on the frame so there is no guessing.
Keep supply and return vents open and clear. Closing vents in unused rooms does not save energy on a forced-air furnace; it raises pressure in the ducts and reduces airflow across the heat exchanger. Keep every register open, keep furniture and rugs off the returns, and vacuum the grilles when you vacuum the floors. Outside, make sure the PVC intake and exhaust pipes on a high-efficiency furnace are clear of leaves, snow, and nests.
Check the thermostat and its schedule. Replace the batteries once a year if it takes them. Confirm the heating schedule still matches how the household lives, and if the furnace seems to run oddly, look at the thermostat before assuming the furnace is the problem. A thermostat mounted near a supply register, a lamp, or a drafty door will cycle the furnace in ways that look like a mechanical fault.
Keep the area around the furnace clear. A furnace needs combustion air and service access. Do not store paint, solvents, or cardboard against it, and leave the front panel reachable. Once a season, take a look at the burner flame through the sight glass or with the lower door off for a moment: steady blue is right, yellow or flickering means it is time to call before the annual visit.
Test your carbon monoxide detectors. Press the test button monthly and replace the units on the schedule printed on the back, since the sensors have a finite life. This is not furnace maintenance exactly, but it is the reason a maintenance schedule matters.
